Case Management

The Case Management Services Department of Hanover Hospital has been in operation since 1966. The Department supports the Mission of Hanover Hospital to provide service excellence in healthcare to patients, co-workers, and the community. The Department has 3 divisions: Social Work, Utilization Management and Chaplaincy Services. Case Management Services are provided through discharge planning, counseling, conferring with insurance companies on behalf of patients, ensuring cost effectiveness and appropriateness of hospitalization and providing for the spiritual needs of patients, their families and employees.

The Case Management Department is open weekdays from 7:30 a.m. – 4 p.m. There is a staff member on call for evenings and weekends. For more information call (717) 633-2123. (Fax: (717) 633-2216) 
 

Social Work

Social Workers have received a bachelor’s or master’s degree in social work. They are part of the treatment team and provide discharge planning which often begins prior to admission. Discharge planning involves linking the patient to resources outside of the hospital that will help them meet their discharge needs. Services with which the Social Workers can assist you are: 

  • Adoption assessments

  • Durable Medical Equipment 
  • Financial assistance 
  • Home Health Care 
  • Home Infusion 
  • Hospice for terminal care 
  • Information and referral 
  • Nursing Home placements 
  • Palliative Care 
  • Support Groups 
  • Tube feedings 
  • Wound care/dressings 

 
Utilization Management and Concurrent Review

The nurses in this area are responsible for a number of functions including the following: 

  • Chart review to determine appropriate level of care. (In-patients, outpatients and observation)
  • Contact with patients insurance company for pre-certification and continued stay review 
  • Assist social workers regarding insurance benefits and pre-certification needed for discharge planning and determining participating providers
  • Explain insurance issues to patients and families as needed
